Commercial Vehicle Operator Record For Carriers Only

Description

Contains a compilation of the carrier's safety performance monitored over a 24-month period with a chronological record of all events entered onto the carriers record during the past five (5) years (maximum).

The record gives the overall profile of the company including events in chronological order back a maximum of five years. Some of the information included is the company's name and address, operating licence number, operator type, CVOR status, original certificate issue date, Ontario Safety Rating, threshold, violation rate, indications of the number of accidents, convictions and inspections, facility audits (if any), and any Ministry interventions within the previous 2 years. The chronological record also provides specific details related to accidents, convictions, and inspections, such as the number of points assigned and the offence description as well as individual accident, conviction and inspection performance thresholds. This will enable the carrier to review the events for accuracy as well as assess its level of safety compliance over the period of the record.

Note: When purchasing an abstract less than the 5 year maximum, the abstract will only reflect details within the time period entered.

A start and end date can be submitted with the order to indicate the events to be included in the print of the Record.

Note: In order to protect the confidentiality of the carrier and to ensure this record is sent only to the carrier, the Commercial Vehicle Operator Record will not be displayed on the screen. Canada Post will deliver the record by regular mail to the carriers mailing address indicated on the record. The requestor will immediately receive a receipt.

You may order a certified or non-certified copy of the CVOR record. A certified copy of a carrier's record is typically used for legal purposes. It has an official seal and is endorsed by the Ministry before mailing. A non-certified copy is usually for the carriers internal use only.

Note: When ordering certified CVOR abstracts, companies older than 2 years will receive a minimum of 24 months and a maximum of 5 years. Companies less than 24 months will get the company’s full history.

Please note that, after January 1, 2006, Carrier Abstracts will only be available to carriers that operate commercial motor vehicles registered in Ontario, United States or Mexico. Others must contact their base plate jurisdiction.
